Admissions

Admission Procedure

This information will help you to gather the materials necessary for registration and will outline the procedures to be followed.
The following documentation will be needed in order to process your application:
  1. Current Student Transcript
  2. Attendance report from present school, if currently enrolled
  3. Most recent report card
  4. Individual Education Plan (IEP) and or Individual Placement Review Committee Report (IPRC)
  5. Canadian birth certificate/Citizenship certificate/Recording of Landing (IMM 1000) passport
  6. Immunization Record, if from outside Brant, Haldimand, or Norfolk counties.
  7. Completed REQUEST FOR OUT-OF-BOUNDARY REGISTRATION FORM, if your residence is not within the Assumption College School's boundaries. (to confirm the school designated for your place of residence call the Brant Haldimand Norfolk Catholic District School Board (519)759-8862).
  8. Proof of custody (if applicable). Legal Custodian is .....
  9. A completed STUDENT REGISTRATION INFORMATION FORM.

It is important to submit your application package as early as possible, since some courses have limited enrolment. When you have the above information collected, please submit it to Mrs. Dickson, in the main office and make an appointment through Mrs. Dickson so that your application can be reviewed. If your application is accepted an appointment will be made, while you are here, with a guidance counselor to register you into courses.
